Payment for Ketamine Services
Ketamine services (including ketamine infusions and ketamine assisted psychotherapy) are not covered by insurance and are available on a self-pay basis only.

Medication Management
The following insurance carriers are accepted for medication management:
Aetna, Anthem Blue Cross and Blue Shield, Blue Cross Blue Shield of Massachusetts, Carelon Behavioral Health, Cigna, CHAMPVA, Oscar, Oxford, Quest Behavioral Health, and UnitedHealthcare
We partner with Headway to make using your insurance simple and hassle-free. Headway handles all of the medication managment billing for your visits, so you can focus on your care—not the paperwork.
Once you schedule your appointment, you’ll receive an email and text message from Headway with a secure link to enter your insurance and payment information. It only takes a few minutes to complete.
✅ Important: Your insurance information must be completed 24 hours before your appointment in order to reserve your spot. If this step isn’t done, your appointment will not be confirmed.
Headway will verify your insurance coverage and co-pay details before your visit. You’ll be responsible for any co-pays or out-of-pocket costs, and Headway will take care of the rest.

IV Hydration and Vitamin Services
IV hydration and vitamin infusions are self-pay services and are not covered by insurance.
These services are provided in collaboration with Rocky Mountain IV Medics, and all payments will be processed directly through them.
Clients may use FSA or HSA funds to cover the cost of treatment.
Self-Pay (Ketamine & Med Management) Policy:
Ketamine Infusion Therapy
$400 per infusion
Initial treatment series: 6 infusions required for new patients
Maintenance: Booster infusions available as needed
Medication Management
New patient evaluation: $275
Follow-up appointments: $150

Payment Terms
Full payment is due at the time of service
Payments are processed securely through SimplePractice
We accept credit cards, debit cards, and FSA/HSA cards
A valid payment method is required to remain on file to confirm and secure appointments
Cancellation & No-Show Policy
We kindly ask for at least a 24-hour notice to cancel or reschedule an appointment
Cancellations made less than 24 hours in advance or missed appointments (no-shows) will be charged a $50 fee
Repeated late cancellations or missed appointments may result in termination of care
Refunds
Fees are non-refundable once services are rendered
Prepaid sessions that are canceled before 24 hours before scheduled appointment time may be refunded or rescheduled at the provider’s discretion
Returned Payments
A $30 fee will be applied to any returned or declined payments (e.g., insufficient funds or expired card)
